Based on our continuing assessment of the city of Milwaukee and the needs of our churches and social agencies, the MMLMS will continue to offer outreach initiatives for 2011 to be funded in part through the Siebert Lutheran Foundation. This year we are more flexible as we are considering your own ideas for outreach activities specifically for your organization. along with giving you ideas that you may want to consider. We did this, as we realize that all of our churches and organizations may have ideas that will work well within their own venue along with the opportunity that we all might also be able learn from each other. We have changed the initiative plan and quarterly report format into an excel form, which will require less information, but more precise information which will be needed for reporting. We have also placed this report into one form so that you will be able to quickly plan and then report on all three initiatives that you are doing with one form that you will be submitting via email.

The outreach programs that you are submitting for funding have been approved or will be approved by your church council or governing board prior to starting.
There is a plan in place at the congregational level that will both recognize and support these outreach programs that you are submitting for funding
There is a designated person or persons responsible for each outreach program that you are submitting for funding.
There is a designated space at the church/school for each outreach program to occur
The overall outreach program is focused in one or more of these areas: Serving, Worship, Education, Evangelism or Fellowship
There is a secondary purpose of each outreach program in developing new leaders or enhancing the skills and abilities of current leaders.

Please fill out the short initial questionnaire and submit to get your planning form. This planning form is due no later than midnight, November 15th, 2010 for consideration of funding for 2011.
