Urban Outreach Initiatives

We are thankful to the Siebert Lutheran Foundation for their support in providing funding for our churches and social agencies so that these outreach activities may take place. Outreach is defined as providing services and programs that bring into our organizations people and neighbors from our surrounding community so that we may develop relationships with them. Our ultimate goal in these relationships is that they get to know Jesus Christ as their Lord and Savior, and experience a deep, growing, and long-lasting, personal relationship that is only available through the work of the Holy Spirit. The programs that you are providing should have this as the number one goal.
This form needs to be filled out and submitted prior to your organization receiving first quarter funding. Please complete the form below or use the Initiative Proposal template to send your response by mail or email to director@mmlms.org. Be advised, one form needs to be completed for each outreach initiative that is receiving funding.When filling out this form, please make sure that you review the Urban Outreach Initiatives page that specifies exactly what information is required. Please note that we are asking for more detailed information on your plans for several reasons:

Evaluation of satisfaction of those who are using your programs

Referral to other churches who may want to consider a specialized program that you are currently doing.

Sharing of information regarding programs that you are using and what has worked or not worked
As you are setting your initial plan for your outreach initiatives, you may change your initiative(s) except for Health and Wellness at this time, but there will be no increase or decrease with your funding. If you have 2 or 3 initiatives, you may also move up to $500.00 from one initiative to another. This can only be done at the initial planning stage, and once your plan is submitted, no other changes will be allowed.
Please make your submissions by January 31.
